SHS Compliance Services
  • Immunization Compliance Nurse:
    • Assists Student with Compliance questions.
    • Collects copies of Immunization Records for Compliance.
    • Collects the Immunization and Screening Record forms.
    • Helps student begin process of removing holds.
  • Immunization and Screening Services
    • MMR Immunizations given at Health Center.
    • Performs TB Screening and Tests.(KU Policy.)

  • This form is to be filled out by a Medical Provider.
  • The Immunization and Screening Record form can be turned in at new student orientation, mailed, faxed or hand carried to Student Health Services.
