MEDICAL RECORD FORMS
KU Immunization Compliance
The University of Kansas requires the following immunizations and/or screenings of its student body:
- MMR: All newly enrolled or re-enrolled students born on or after January 1, 1957
must show proof of TWO vaccinations for Measles, Mumps and Rubella. History of the disease,
unless accompanied by positive titer, is not acceptable.
- MENINGITIS: All students living in university owned group housing are required
to be vaccinated for meningitis or to sign a waiver indicating that they refuse to receive the vaccine.
- TUBERCULOSIS: All newly enrolled or re-enrolled international students are
required to be screened for tuberculosis by Student Health Services upon arrival on campus and prior to enrollment.
Failure to comply with the requirements described above will result in an enrollment hold. For more information, please visit our

REQUIRED HEALTH FORMS
Both forms are required to be filled out and returned to Student Health Services. These forms can be turned in at any New Student Orientation session, mailed, faxed or hand carried to Student Health Services.

Policy to Documentation of Illness or Injury (Class Note)
SHS can provide documentation of treatment. The policy is as follows:
- Students who are incapacitated for three or more consecutive days
due to illness or injury can request a Documentation of Illness/Injury
Statement from the SHS medical professional who either treated the student,
or receive appropriate documentation from another medical provider.
- Students who have illnesses or injuries of shorter duration and received care
at SHS can obtain a copy of their medical record containing information about
their visit by submitting an
Authorization for Use/ Disclosure form to the SHS
Records and Registration Department. The processing of this request will be
completed within 10 working days.

Tuberculosis Screening for International Students
All newly enrolled or re-enrolled international students are required to have
tuberculosis (TB) screening upon arrival at KU.
A TB blood test will be done during International Student Orientation physicals,
unless documented results are presented for a blood test performed in the U.S.
within the past 12 months.
Documentation must be presented for any prior treatment for latent or active
tuberculosis and a chest X-ray will be required.
Chest X-rays will also be required for any student with a positive or indeterminate
TB blood test, unless documentation of a normal X-ray performed in the U.S. within
the past six months is presented.
All newly-enrolled international students are required to complete the entire
tuberculosis screening process. Failure to do so will result in dis-enrollment and
subsequent notification to the Office of International Students and Scholars that
you are no longer enrolled.

Tuberculosis Screening Policy for Other Students
Certain programs, such as the School of Pharmacy and Department of Athletics,
require their students to be screened for tuberculosis as well. In those cases,
the students will be informed directly by the program as to their compliance
requirements.
