SHS - Documentation of Illness or Injury Policy
(Class Note)

In an effort to improve customer service and to continue to meet the medical needs of our patients, Student Health Services has modified its policy on documenting class/work absences for extended illness or injury.

Per patient request, class notes will be given when the following two conditions are met:

• Patients have received care/treatment by Student Health Services or have documentation of treatment provided by another healthcare provider.


AND


• Patients have had three or more consecutive days of absence due to extended illness or injury.

Patients who are not eligible for a class note but would like documentation of visits at Student Health Services, may obtain a copy of their medical record by completing an AUTHORIZATION FOR USE/DISCLOSURE form. Processing of this request could take up to ten days.
