At the student's request, SHS can provide documentation for missed class/work time due to treatment for illness or injury is as follows:

1. Extended Absence. Students with three or more consecutive days of absence that have received treatment by SHS (or SHS has received documentation of treatment elsewhere) may request a physician memorandum/class note from the SHS clinician who is providing treatment.


2. Shorter Absence. Students who have absences of less than three consecutive days and have received treatment at SHS can obtain a record of their visit by completing and submitting an Authorization for Use/ Disclosure form to the SHS Records and Registration Department. The processing of this request may take up to 10 days.

Please be aware that SHS can provide documentation, but only the professor/employer has the authority to excuse an absence.
